As a patient receiving mental health or substance abuse treatment, you have the right to:

2. Confidentiality and Privacy

You have the right to expect that your personal and medical information will be kept confidential in accordance with HIPAA and applicable state and federal laws.

3. Informed Consent

  • Receive clear explanations about your diagnosis, treatment options, and expected outcomes.
  • Ask questions about medications, therapy, and treatment plans.
  • Participate in decisions regarding your care.

4. Safe and Supportive Environment

You have the right to receive care in a safe, professional, and therapeutic environment free from discrimination, harassment, or abuse.

5. Access to Treatment

  • Receive appropriate and timely care.
  • Be informed of available services and alternatives.
  • Request a second opinion when appropriate.

6. Participation in Treatment Planning

Patient Responsibilities

To ensure effective care and a safe therapeutic environment, patients are expected to:

1. Provide Accurate Information

  • Providing complete and accurate medical, psychiatric, and substance use history.
  • Informing providers about changes in your condition or medications.

2. Participate in Treatment

  • Actively participating in therapy, counseling, and recovery activities.
  • Following agreed-upon treatment plans to the best of your ability.

3. Keep Appointments

  • Arriving on time for scheduled appointments.
  • Notifying the office in advance if you need to cancel or reschedule.

4. Respect Staff and Other Patients

  • Maintain respectful communication.
  • Refrain from disruptive, threatening, or violent behavior.

5. Follow Program Policies

  • Adhering to facility rules and safety guidelines.
  • Following medication instructions and clinical recommendations.

6. Financial Responsibility

  • Providing accurate insurance information.
  • Paying applicable co-payments, deductibles, or balances as required.

7. Support a Safe Environment

  • Avoid bringing illegal substances, weapons, or prohibited items into the facility.
  • Notify staff if you feel unsafe or observe unsafe conditions.
